Aged Care Package Scheme
At Our Health Tribe, we’re committed to helping older Australians live independently and maintain their well-being. The Aged Care Package Scheme (also known as the Home Care Packages program) is a government-funded initiative designed to support seniors who want to remain in their homes for as long as possible. Through this scheme, eligible individuals receive tailored funding that can be used to access a range of services—including those we provide at Our Health Tribe—to boost comfort, mobility, and overall health.

What Is the Aged Care Package Scheme?

Government-Funded Support: The Australian Government subsidises a portion of in-home care costs, with four different package levels (Level 1–4) based on assessed need.

Personalised Care Plans: Once you’re approved, a Care Plan is developed in consultation with you (and your support person, if you wish). This plan outlines the services you need—be it health services, domestic assistance, or allied health support.

Flexible Use of Funds: Funding is allocated directly to you, and you choose a provider (or providers) to deliver the services in your Care Plan. At Our Health Tribe, we can work with you (and your approved package) to deliver the therapies you need.
Who Is Eligible?

Age Requirement: Generally, you must be aged 65 or older (50+ for Aboriginal or Torres Strait Islander people).

Assessment by ACAT/ACAS: To access the scheme, you first need an assessment from the Aged Care Assessment Team (ACAT) or Aged Care Assessment Service (ACAS) in your state or territory. They’ll determine your level of care needs.

Australian Citizen or Permanent Resident: You must reside in Australia and hold appropriate residency status.

Living at Home: You need to be living in a private residence (own home, family home, or rental) or in a retirement village—as long as you’re not already in a residential aged care facility.

Undergo the Assessment

An assessor (often an occupational therapist, nurse, or social worker) will visit your home to discuss your health, mobility, social support, and daily-care needs.

They recommend which level of Home Care Package (Level 1–4) is most appropriate.

Receive Approval & Package Level

If approved, you’ll be placed on a national queue. Wait times vary, but you can nominate providers (like Our Health Tribe) to deliver your approved services.

Choose Your Provider

Once your name reaches the top of the list, you receive a formal offer. You then select a Home Care Package provider (also called a “registered provider”) to manage your funds and coordinate services.

Our Health Tribe can be nominated as your preferred allied health provider, working alongside your package coordinator to deliver therapies.

Massage Therapy & Lymphatic Drainage
Therapeutic massage to relieve muscle tension, promote circulation, and support lymphatic flow, which can reduce swelling and improve comfort. -
Health Coaching & Lifestyle Planning
One-on-one guidance to set realistic health goals, manage chronic conditions, and stay motivated through tailored action plans. -
In-Home Visits (Where Available)
For clients who have difficulty travelling, we can arrange home visits—subject to package funding and your Care Plan.
Tip: Always check with your Home Care Package coordinator or your My Aged Care portal to confirm which services are included under your approved package level. We can liaise directly with your coordinator to ensure seamless service delivery.

Frequently Asked Questions
1. Which package level covers osteopathy and massage therapy?
Level 2 (Low-Level Care Needs): You may receive occasional allied health visits—typically one or two osteopathy or massage sessions per month, depending on your overall Care Plan and budget.
Level 3 (Intermediate Care Needs): Offers a larger budget that can support a regular schedule (e.g., weekly or fortnightly) of osteopathy and/or massage therapy.
Level 4 (High-Level Care Needs): Provides the most funds, allowing for ongoing, frequent allied health services (such as weekly osteopathy or massage) without needing to pay a significant gap.
Tip: The ACAT/ACAS assessor will recommend the level based on your assessed need for pain management, mobility support, or complex conditions.
2. Can I access all Our Health Tribe services through my Home Care Package?
Most of our holistic and allied health therapies—Osteopathy, Naturopathy, Hypnotherapy, Remedial Massage, and Health coaching—can be funded if they’re included in your Care Plan. We liaise with your package coordinator to confirm which services align with your approved funding.
3. What if my package level doesn’t fully cover the cost of a service?
You have two options:
Pay the Gap: Book the therapy you need and cover any shortfall between package funding and the provider’s fee.
Adjust Your Care Plan: Work with your coordinator to reallocate funds—prioritising the services most critical to you—or consider requesting a re-assessment to move to a higher level.
4. How long does it take to get approved for a Home Care Package?
Wait times vary by region. After your ACAT/ACAS assessment, it can take anywhere from a few weeks to several months before you receive a formal package offer. Once approved, you can retroactively apply funding to any interim private sessions (subject to package rules).
5. My loved one has mobility challenges — can Our Health Tribe come to the home?
Yes. If in-home visits are approved in your Care Plan, we can arrange home-based osteopathy, massage, or other therapies. Availability depends on your package funding and location (e.g. Noosa region). Let us know when you book, and we’ll confirm home-visit options.
6. How do I choose Our Health Tribe as my provider?
Complete your ACAT/ACAS assessment via My Aged Care (1800 200 422).
When you receive your package offer, inform your package coordinator that you wish to nominate Our Health Tribe for any allied health services.
Contact us to schedule your first appointment—either in-clinic or (if approved) at home—and we’ll coordinate directly with your coordinator to finalise your Care Plan.
